Job summary

AdvisaCare Hospice is expanding in Livonia, MI, with Troy as the parent agency, offering PRN/contingent Hospice RN roles to support Wayne and Monroe counties. You will provide compassionate hospice care, conduct supervisory visits, and contribute to interdisciplinary team meetings to develop and revise patient/family plans of care.

The position emphasizes emotional support for patients at the end of life, with on-call field work and collaboration with the Hospice team to ensure high-quality care.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Be prepared for field visits daily and on-call requirements.

Responsibilities

• Initial and on-going assessment of patient needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice policies and Medicare requirements.
• Educate patient/family on disease process and end-of-life care.
• Supervise paraprofessionals providing services to patient.
• Prepare clinical and progress notes; participate in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current processes and technology within the organization.
