Job summary

AdvisaCare is expanding its hospice services from the Livonia office, serving the surrounding areas, with the Troy agency as the parent office. We seek an RN who values hospice care and will provide compassionate, dignified end-of-life support to patients and families.

Responsibilities include supervisory visits, collaboration in IDG conferences, and ongoing development of the patient/family plan of care. A Michigan RN license and hospice experience are required to ensure high-quality care.

Qualifications

• Empathy and compassionate communication with patients and families.
• Ability to educate patients and families on disease progression and care planning.
• Strong interpersonal skills to coordinate with interdisciplinary teams.

Responsibilities

• Provide emotional support to patients to ensure dignity and comfort in their final days.
• Conduct supervisory visits to observe the performance of team members and provide feedback in IDG meetings.
• Develop, review, and revise the patient/family plan of care with input from the care team.
• Maintain high customer satisfaction with clients, families, and caregivers.
• Perform field visits and participate in daily office meetings as needed.
