Job summary

AdvisaCare Hospice is seeking a dedicated RN to serve as a Hospice Home Care Case Manager in the Southfield and Waterford areas, servicing Oakland County. You will provide compassionate, one-on-one home-based hospice care, guiding patients and families with dignity and comfort—spending the time and attention needed in moments that matter most.

As a full-time Hospice RN, you will perform supervisory visits, contribute to interdisciplinary team planning, and ensure adherence to hospice policies

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience preferred.
• Empathetic approach to patients and families.

Responsibilities

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental).
• Implement and adjust the individualized plan of care.
• Educate patients and families on disease process and end-of-life care.
• Provide supervisory visits and support to hospice team and IDG meetings.
