Job summary

AdvisaCare Hospice is expanding its Livonia office, adding a Hospice RN role to support Wayne and Monroe County clients. The PRN/contingent position emphasizes compassionate patient care, supervision of paraprofessionals, and coordination with interdisciplinary teams to uphold dignity and comfort at end of life.

The ideal candidate holds a current Michigan license, has hospice experience, and can participate in field visits daily with on-call availability.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily and attend weekly office meetings.
• Full-time on-call availability.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance.
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care.
• Supervision of paraprofessionals providing services to patient.
• Preparing clinical and progress notes, participating in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with our clients, families and caregivers.
• Communication of information using current process and technology within organization.

Are you looking for a challenging and rewarding position as a PRN/CONTINGENT Hospice RN? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family with our Troy office location being your Parent Agency servicing our clients for the Wayne and Monroe County.

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.
