Job summary

Optum's Deaconess Hospice in Laurel, MS is seeking a Hospice Registered Nurse (PRN). You will assess patients, coordinate care plans, and work with the IDG to deliver compassionate hospice services.

Qualifications

• Current unrestricted RN licensure in the state of practice (MS).
• 1+ years of clinical nursing experience.
• Current driver's license and access to reliable transportation.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and responses to hospice care.
• Collaborates with patient/family, attending physician and IDG daily.
