Job summary

St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to deliver holistic, in-home patient care, collaborate with a care team, and lead meaningful end-of-life conversations.

You will develop personalized care plans, administer medications, educate families, and participate in on-call duties to ensure seamless care transitions. Qualifications

• 1+ years of recent acute care experience required; hospice/home health experience preferred.
• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed, ensure symptom management.

Responsibilities

• Deliver holistic, individualized patient care in home setting.
• Collaborate with care team to develop and implement plan of care.
• Lead end-of-life conversations with patients and families.
• Coordinate seamless care transitions and on-call coverage.
