Job summary

Live Well Homecare Agency, LLC seeks a Registered Nurse (RN) to provide compassionate end-of-life care at a 16-bed Hospice House. The RN will assess patient needs, develop individualized care plans, and coordinate with a multidisciplinary team to ensure high-quality, culturally sensitive, patient-centered care.

Responsibilities include evaluating patient conditions, communicating with families, and updating care plans as conditions change.

Qualifications

• RN license required.
• Minimum of 2 years of nursing experience; hospice preferred.
• Valid Driver's License and registration with the Family Care Safety Registry.
• Preferred: Bachelor's Degree in Nursing or Home Health experience (2-5 years).
• Strong assessment, communication, and critical thinking skills; ability to work collaboratively in a team.

• Assess patient preferences, barriers, and needs including emotional, spiritual, and cultural factors.
• Develop, implement, and document personalized care plans.
• Coordinate care across teams, promote respectful interactions, and advocate for patient and family needs.
• Monitor changes in patient condition, update care plans, and communicate effectively with families and team members.
• Ensure adherence to safety and quality standards.
