Job summary

Four Pillars Hospice is seeking a Registered Nurse (RN) to provide compassionate nursing care to hospice patients in their homes. You will collaborate with the interdisciplinary team to assess needs, implement plans, and ensure quality end-of-life care.

Responsibilities include patient assessments, developing care plans, educating patients and families, coordinating with physicians, and participating in QAPI and in-service education. A current RN license and hospice experience are required.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelors School of Nursing.
• Current RN license in the state of practice and/or compact license eligibility.
• Hospice experience required.
• Understanding of hospice regulatory requirements, documentation standards, and compliance.
• Strong clinical, communication, and interpersonal skills.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Perform initial and ongoing patient assessments.
• Develop, implement, and update individualized plans of care.
• Coordinate care with the Interdisciplinary Team, physicians, patients, and families.
• Provide patient and caregiver education regarding disease process, medications, end-of-life care, and safety.

• Document clinical findings, interventions, and patient progress accurately.
• Report changes in patient condition to physicians and team members.
• Practice infection control and standard precautions.
• Participate in QAPI and hospice in-service education programs.
• Provide on-call services as assigned.
• Promote hospice philosophy and quality patient care.

The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.
