Job summary

CarePartners Health Services in Asheville, NC, is seeking a Primary Case Manager RN to provide nursing care and clinical case management in hospice. You will assess patients and families, adjust plans with the interdisciplinary team, and coordinate services to relieve symptoms and support quality of life.

Qualifications

• Diploma, associate, or bachelor degree in nursing.
• Current RN licensure in North Carolina with compact license eligibility.
• Basic life support (BCLS) certification.
• Valid driver's license.

Responsibilities

• Provide nursing care and clinical case management to hospice patients and families.
• Complete assessments and modify plans of care with the hospice interdisciplinary team.
• Coordinate services with the IDG to support patient care and transitions.
