Job summary

Treasure Coast Hospice seeks a Registered Nurse to plan and deliver skilled nursing care in the Fort Pierce area, providing palliative and supportive care to patients and families. You will perform comprehensive assessments, develop care plans with the interdisciplinary team, and document care in the EMR, while maintaining professional boundaries and confidentiality.

Requirements include a Florida RN license, CHPN within 12 months, CPR, and a valid Florida driver's license with auto insurance.

Qualifications

• Current and active Florida RN license.
• Must be CHPN within 12 months.
• Current and valid Florida driver’s license and automobile insurance.
• Current CPR certification required.
• Minimum of one year RN experience with physical assessment.
• Prior oncology/home health/hospice or palliative care experience preferred.
• Able to plan, organize and prioritize work.
• Self-directed and able to work independently in the field.
• Basic computer skills and EMR software experience preferred.

Responsibilities

• Plan and deliver skilled nursing care and supervision per policy.
• Perform comprehensive patient assessments and update care plans.
• Administer medications and document with patient progress.
• Communicate with IDT and family to align goals of care.
• Maintain confidentiality and professional boundaries.
• Supervise CNAs and LPNs as part of the team.
• Participate in compliance and program development initiatives.
• Ensure accurate documentation of visits and outcomes.
