Job summary

Constellation Health Services seeks a compassionate Per Diem Registered Nurse to provide hospice care for patients in home settings, assisted living facilities, and skilled nursing facilities. You will conduct comprehensive assessments, coordinate plans of care with families and a multidisciplinary team, and document accurately in our system.

Qualifications include an accredited RN program graduate, active state RN license, 1–2 years of RN experience, and hospice care preference.

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse licensed in the state of employment.
• At least 1–2 years of RN experience is a MUST.
• Previous hospice care strongly desired.
• Basic knowledge of iOS operating system.

Responsibilities

• Provides skilled nursing care including administration of medications and treatments as prescribed in the plan of care for patients in their home setting.
• Performs comprehensive assessments and coordinates the plan of care with all disciplines involved, including family and primary caregivers.
• Monitors and reports patient status to nursing supervisor and attending physician when changes occur.
• Participates in weekly Interdisciplinary Team Meetings and provides updates on patient status.
• Maintains and documents all records in compliance with agency standards; uses a computer-based documentation system.
• Weekend and holidays coverage as needed.
