Job summary

Constellation Health Services is seeking a compassionate PER DIEM HOSPICE Registered Nurse to provide in-home and facility-based hospice care to patients and families. You will perform comprehensive assessments, administer medications, and coordinate care with interdisciplinary teams.

The role requires 1–2 years of hospice or home health experience, RN license in the state, and willingness to cover weekends and holidays as needed. A supportive, family-centered environment awaits you.

Qualifications

• Graduate of an accredited RN program.
• RN license in state of employment is required.
• 1–2 years of RN hospice or home health experience is preferred.

Responsibilities

• Provides skilled nursing care including medications and treatments per physician plan in patient homes or facilities.
• Performs comprehensive assessments and coordinates care with involved disciplines.
• Observes, evaluates and reports patient status to nurse supervisor and physician.
• Participates in weekly Interdisciplinary Team Meetings and updates on patient status.
• Maintains and processes documentation per agency standards using the EHR system.
• Provides input to Home Health Aide Supervisor for hospice aide evaluation.
• Occasional weekend and holiday coverage as needed.
