By The Bay Health is seeking a Hospice Team Nurse (RN) to provide coverage for Nurse Case Managers, coordinating with Team Manager for patient care needs. This full-time role requires California RN licensure and offers competitive pay and benefits.

The position emphasizes nursing process, care coordination, and case management within a hospice/home health setting, with responsibilities to assess and plan patient/family needs and ensure timely care delivery.

Competenze

• Experience using the nursing process to assess and plan patient care.
• Ability to coordinate services with the transdisciplinary team.
• Experience with case management and referrals to related services.
• Knowledge of home health and hospice settings is preferred.
