By The Bay Health in Marin County seeks a Hospice Team Nurse (RN) to support Nurse Case Managers and coordinate patient care. Schedule is four days a week, 8:30am–5:00pm, in Marin County. The role requires California RN licensure and experience in home health or hospice. CDPH vaccine requirements are followed.

Formación

• One year of professional nursing experience in the past three years or a BSN from an accredited School of Nursing.
• Experience in home health and/or hospice setting preferred.
• Current license as a registered professional nurse in the State of California.

Responsabilidades

• Use the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions.
• Coordinate patient/family services and prioritization with the transdisciplinary team.
• Use a case management approach and refer to other services as needed.
• Coordinate plan of care implementation and ensure care coordination and case management for each patient.
