By The Bay Health, a nonprofit established in 1975, is a hospice provider offering home-based care across Skilled Home Health, Palliative, Adult Hospice, and Pediatric Care. The Admissions/Team Nurse position supports Nurse Case Managers, coordinates admissions, and ensures patient-centered care within a team framework.

The role requires an active California RN license, with preferred Public Health Nursing or HPNA certifications, and visits in San Francisco and surrounding counties.

Formación

• Associate's or Bachelor’s degree in nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Responsabilidades

• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.
