Job summary

By The Bay Health in San Francisco is seeking an Admissions/Team Nurse to coordinate admissions and provide care for hospice patients. The role covers nurse case management, in-person visits, and collaboration with a transdisciplinary team in a home-based or facility setting.

Ideal candidates hold an Associate’s or Bachelor’s degree in nursing and have experience in acute care, home health or hospice. California RN license and public health nursing certification are preferred.

Qualifications

• Associate’s or Bachelor’s degree in nursing from an accredited School of Nursing.

Responsibilities

• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.
