Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license.
• Admissions and On-call experience preferred.
• Strong clinical and communication skills; ability to triage and respond to urgent needs.

Responsibilities

• Provide hospice care and services to assigned patients in homes, facilities, or hospitals.
• Complete hospice admissions and conduct patient visits as needed.
• Answer calls from patients, families, and customers; document and coordinate care.
• Perform symptom management visits, handle admissions, and respond to after-hours needs.
