Job summary

Brighton/Suncrest Hospice is seeking a hospice RN to work on-call. We have both Full-Time and PRN available. Must have hospice experience.

The RN will provide care to a group of hospice patients in homes, facilities, hospitals, and will handle admissions and visits. You will be part of a CHAP-certified hospice team that emphasizes high-quality care and favorable staff-to-patient ratios. The after-hours on-call schedule is seven days on/off, with travel across the service area and driving

Qualifications

• Current Registered Nurse (RN) licensure in good standing with one year of experience.
• Admissions and On-call experience preferred.
• Works well within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Independent and capable of day-to-day activities including phone, email and written communications, meetings with healthcare professionals.

Responsibilities

• Provide care and services for a group of hospice patients, visiting patients at private homes, assisted living facilities, and hospitals.
• Complete hospice admissions and patient visits as needed; respond to calls from patients, families and customers.
• Deliver symptom management, end-of-life care, and admissions; travel across the service area as required.

Details

The Registered Nurse (RN) will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities and hospitals. As a RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. This position will be after hours on-call working seven days/nights on and seven nights/days off.

Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license


• Admissions and On-call experience preferred


• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.


• Work closely with patients on a 1-1 level but get strong support from a patient care team.


• Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.


• Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)


• Ability to learn an electronic medical records system.


• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
