Brighton Hospice is seeking an experienced Registered Nurse (RN) to join our on-call team for night shifts, providing compassionate hospice care in patients' homes, SNFs, and assisted living facilities. You will collaborate with an interdisciplinary team to develop and adjust care plans, ensuring high-quality, patient-centered support.

The role emphasizes reduced caseloads and the chance to earn bonuses through on-call visits, with full-time pay and mileage reimbursement.

Qualifications

• RN license in good standing in the state of practice
• Ability to collaborate with interdisciplinary care teams
• Strong time management and organizational skills
• Comfort learning and utilizing EMR systems
• Reliable transportation to travel throughout the service area

Responsabilités

• Provide compassionate hospice care in patients' homes and facilities
• Collaborate with nurses, social workers, chaplains, CNAs and volunteers
• Develop and implement individualized care plans for physical, emotional, and spiritual needs
