Job summary

Treasure Coast Hospice is seeking a RN PD Nights On Call / Triage to provide skilled nursing and oversight in a 8am-8pm schedule across Martin, St Lucie and Okeechobee counties. You will triage calls, cover crisis care, and support inpatient units as needed, delivering compassionate, evidence-based care under Florida statutes and organizational policies.

Must hold a Florida RN license, CHPN within 12 months, and have 1 year of RN experience (ER/Charge Nurse preferred).

Qualifications

• Active Florida RN license.
• CHPN within 12 months of hire.
• Florida driver’s license and auto insurance.
• Minimum 1 year RN experience, preferably ER or as a Charge Nurse.
• Experience in oncology, home health, Hospice or palliative care preferred.
• BLS CPR certification required.
• English reading/writing/speaking required; bilingual preferred.

Responsibilities

• Uphold hospice standards for patient visits and documentation.
• Provide triage via phone and in-person visits as needed.
• Coordinate with IDT to review and revise care plans.
• Lead and direct RN, LPN, and CNAs as required.
• Educate patients, families, and caregivers about interventions and meds.
• Assess physical, emotional, spiritual, and psychosocial needs.
• Maintain EMR documentation of care and events.
• Respond to emergencies and support crisis care as needed.
