Job summary

VIA Health Partners in Charlotte, NC seeks a compassionate Registered Nurse to deliver end-of-life care at Hospice House Aldersgate. You will assess patients, coordinate care, educate families, and collaborate with the interdisciplinary team to meet care goals.

Qualifications

• Diploma or associate degree in nursing required; Bachelor’s Degree preferred.
• Current, active, and unrestricted RN license (multistate).
• Current CPR certification; CHPN preferred.
• Minimum 1 year related experience for IPU and 2 years for other nursing positions; hospice experience preferred.
• Computer proficiency required.

Responsibilities

• Coordinates and delivers patient/caregiver care.
• Performs nursing assessments to manage patient/caregiver needs.
• Develops, implements, evaluates, and updates the plan of care to ensure continuity.
• Educates patients and families; provides caregiver instruction.
• Communicates with the IDG and other health professionals to ensure continuity of care.
• Maintains infection control and follows universal precautions.
