Qualifications

• Master’s Degree in Social Work from a CSWE-accredited program.
• Licensed Clinical Social Worker is required.
• Medical social work or hospice experience preferred.
• Computer proficiency is required.

Responsibilities

• Conduct psychosocial assessments of patients, caregivers, and families to identify emotional, social, and environmental needs related to illness and end-of-life care.
• Develop, implement, and evaluate plans of care addressing counseling, education, and resource/referral needs while maintaining patient dignity.
• Provide culturally sensitive care by considering spiritual and cultural beliefs and collaborating with the IDT.
• Document assessments, care plans, and interventions per HPCCR guidelines.
• Lead or participate in family meetings and coordinate with IDT and HPCCR departments to meet patient goals.
