Job summary

Grace Healthcare Services is seeking a Hospice Registered Nurse (RN) to join our Interdisciplinary Team in Morris County, NJ. The role involves delivering skilled nursing care, coordinating with an IDT, and supporting patients and families throughout their hospice journey.

The schedule is Mon-Fri 8:30-5:00 pm, with a competitive compensation package including benefits such as health, dental, vision, life insurance, PTO, paid sick time, tuition reimbursement, and 401K with company match.

Qualifications

Valid New Jersey or Multi-State RN license.
Minimum of two years of nursing experience in acute-care settings (medical/surgical/oncology).
Strong clinical practice with customer service, excellent communication, and ability to work independently.

Responsibilities

Deliver skilled nursing care per individualized care plans to terminally ill patients.
Collaborate with physicians, social workers, chaplains, nursing assistants, and others to adjust care plans.
Educate and support patients' families and primary caregivers; provide emotional support.
Serve as liaison between patients, families and healthcare providers; advocate for patients' wishes.
Travel daily to patient locations and facilities as assigned.
