Three Oaks Hospice is seeking an RN to provide skilled nursing care in patients’ homes, coordinating with the interdisciplinary team and families to ensure high-quality end-of-life care. The role involves assessment, care planning, and patient education across diverse ages and needs.

The ideal candidate holds an RN license, 2+ years of clinical RN experience (hospice/home health preferred), and CPR certification. Strong communication and organizational skills are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.

Responsabilités

• Perform initial and ongoing comprehensive assessments of patients’ needs in their home.
• Coordinate care with the interdisciplinary team and family; update plan of care as needed.
• Educate patients and families about disease process, end-of-life care, nutrition, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
