Job summary

Brighton/Suncrest Hospice seeks an experienced Registered Nurse (RN) to deliver compassionate hospice care across home, SNF, and assisted living settings. You will work with an interdisciplinary team to create and adjust care plans, while maintaining a patient-centered approach and ensuring high-quality end-of-life care.

Qualifications

• Strong collaboration with interdisciplinary care teams.
• Ability to work independently with daily scheduling.
• Excellent time management and organizational skills.
• Comfort with electronic medical record (EMR) systems.

Responsibilities

• Provide compassionate, individualized hospice care in patients' homes and facilities.
• Collaborate with nurses, social workers, chaplains, CNAs, and volunteers to develop care plans.
• Maintain lower clinician caseloads and higher staff-to-patient ratios to ensure quality care.
