Brighton Hospice in Denver, CO is seeking an experienced Registered Nurse to provide compassionate hospice care in patients' homes, SNF facilities, and assisted living communities. You will collaborate with nurses, social workers, chaplains, CNAs, and volunteers to develop personalized care plans and ensure high-quality care in a lower caseload setting.

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary care teams.
• Ability to work independently while effectively managing daily schedule.
• Excellent time management and organizational skills.
• Comfort learning and utilizing EMR systems.
• Reliable transportation, valid driver's license, and current auto insurance.

Responsibilities

• Provide compassionate hospice care wherever patients call home.
• Collaborate with an interdisciplinary team to develop care plans.
• Maintain quality with balanced clinician caseloads and strong relationships.
• Offer support to patients and families addressing physical, emotional, and spiritual needs.
• Document visits and care activities and coordinate with the team.
