Compassus is seeking a Hospice Clinical Liaison to promote our care philosophy and facilitate admissions from hospitals and communities. You will work with referral sources, patients, and families to ensure accurate information and smooth transitions to hospice services.

The role requires an RN license and 2–3 years of related experience, with strong communication skills and the ability to collaborate across departments to meet growth targets.

Qualifikationen

• Ability to read, analyze, and interpret general business materials.
• Strong written and verbal communications in a healthcare setting.
• Ability to understand and articulate hospice services and processes.
• Clinical experience in hospital or long-term care settings.

Aufgaben

• Attend meetings, educational programs, and community events as requested.
• Explain hospice services to patients and families to foster understanding.
• Coordinate admissions, gather signatures, and relay information to Admissions.
• Coordinate discharge from hospital to home and arrange DME, meds, and transport.
• Educate hospitals and facilities on general inpatient care purpose and process.
