Job summary

Gentiva is seeking an experienced Hospice Clinical Liaison to streamline intake and referrals, coordinate admissions, and strengthen relationships with referrals and healthcare partners. The role involves patient-facing evaluations, education, and comprehensive documentation across clinical and payer requirements.

The ideal candidate is a licensed RN with 3–5 years in hospice, BSN preferred, and a track record of collaborating with discharge planners, case managers, and physicians to ensure

Qualifications

• Licensed nurse with hospice experience required.
• Minimum three years in hospice care.
• Discharge planning, case management, or educator experience preferred.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians and families.
• Educate patients and families on hospice care and processes with clarity and compassion.
• Complete documentation and coordinate intake/admission, ensuring legal, clinical, and payer requirements.
• Serve as liaison between referral sources and hospice team for seamless handoffs and rapid responses.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify outreach opportunities.
