Job summary

Amedisys is seeking an Admissions RN for full-time days in Nashville. You will coordinate hospice admissions, assess patients, and develop initial care plans with the interdisciplinary team to ensure comfort at home.

Responsibilities include on-call coverage, accurate documentation, and ongoing communication with physicians. Requires an active RN license in the state and CPR certification; prior hospice experience is a plus.

Qualifications

• Current RN license (state-specific)
• One year of clinical RN experience
• Current CPR certification

Responsibilities

• Coordinate admissions of hospice patients with initial assessments.
• Establish initial plan of care with physicians and interdisciplinary team.
• Provide skilled interventions and instructions to patients and caregivers.
• Act as liaison between disciplines during admission process.
• Maintain and document patient care progress records accurately.
• Communicate changes in patient condition to care team and physicians.
• Perform on-call responsibilities as assigned.
• Undertake other duties as assigned.
