Zusammenfassung

UnitedHealth Group, through its Optum family, is seeking a PRN RN Hospice in Egg Harbor Township, NJ to provide in-home hospice care. Travel within Atlantic, Cumberland, Gloucester, Cape May, Ocean, and Camden counties within a 1-hour radius from Egg Harbor is required.

Responsibilities include initial nursing evaluation within 48 hours, care planning, collaboration with the IDG, and family teaching. Must have NJ RN licensure; on-call evenings/weekends; benefits and career growth opportunities

Qualifikationen

• Current and unrestricted RN licensure in New Jersey.
• Driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• CPR certification or ability to complete within 90 days of hire.

Aufgaben

• Make the initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and reassess as needed (every 15 days).
• Initiate and coordinate the plan of care; document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes regarding patient condition.
