Job summary

Optum, part of UnitedHealth Group, is seeking a Home and Community Based Services Aide to support hospice care in New Jersey. You will assist with personal care and daily living activities for clients in their homes, enabling them to stay safely at home.

Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation.
• CNA or CHHA certification in NJ required.
• 6+ months of home care experience preferred.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
