Zusammenfassung

Bella Terra Home Health and Hospice is seeking a Registered Nurse to join our Inland Empire hospice team. The RN will plan, coordinate, and deliver patient-centered home health care with an emphasis on comfort and education, collaborating with families and clinicians.

Active CA RN license with at least 1 year of direct patient care experience (hospice preferred) and CPR certification are required. A valid driver's license and reliable vehicle are also necessary to support in-home visits.

Qualifikationen

• Active CA RN license is required.
• Minimum of one year of direct patient care experience; hospice experience preferred.
• Current CPR certification required.
• Must have a valid driver’s license and reliable insured vehicle for in-home visits.

Aufgaben

• Develop and implement the plan of care with the patient and caregivers.
• Initiate nursing procedures and provide physician-ordered services.
• Provide patient and caregiver education.
• Identify discharge planning needs and assist with transition arrangements.
• Ensure equipment and services are available for the care plan.
• Supervise LPNs/LVNs and HHAs as part of the care team.
