Job summary

Elara Caring is seeking a Registered Nurse to provide comprehensive home health care. You will assess patients, develop and update plans of care, and coordinate with physicians, branch leadership, and the care team to ensure safe, quality care.

You will educate patients and families about hospice concepts, manage symptoms, and collaborate with interdisciplinary teams to support patient goals in home settings.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program.
• 1+ year of clinical nursing experience.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Assess patients’ physical, psychological, social, and spiritual needs and integrate findings into the Plan of Care.
• Complete initial evaluations and reassess nursing needs during follow-up visits.
• Monitor patient condition and promptly communicate significant changes to the care team.
• Update the Plan of Care as patient needs change and coordinate revisions with the care team.
• Educate patients and families on the Plan of Care and hospice philosophy, including pain and symptom management.
