Bella Terra Home Health and Hospice is seeking a Registered Nurse for hospice care in the Inland Empire. The RN will plan, coordinate, and deliver home health services, collaborating with the patient, family, and the care team to ensure compassionate, high-quality care.

Responsibilities include developing individualized care plans, patient education, discharge planning, and supervising care staff to maintain seamless home-based services.

Qualifications

• Active California RN license required.
• Minimum 1 year direct patient care experience; hospice setting preferred.
• CPR certification current and valid.

Responsabilités

• Develop and implement the plan of care in partnership with patient and caregivers.
• Provide preventive and rehabilitative nursing services as ordered.
• Educate patient and caregiver on disease, treatment, and home care.
• Identify discharge planning needs and coordinate before discharge.
• Arrange equipment and services needed for home care.
• Supervise LPNs/LVNs and HHAs as assigned.
