Grace Healthcare Services in New Jersey is seeking a compassionate Hospice Registered Nurse (RN) to join our Interdisciplinary Team. You will assess and address the needs of patients and families, initiating interventions at admission and maintaining a continuous care plan throughout the journey.

The role requires an NJ or multi-state RN license and at least two years of acute-care nursing experience, with strong communication and independent work skills, plus a willingness to travel to patient

Role Overview

As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.

Key Responsibilities

• Deliver skilled nursing care as outlined in individualized care plans, ensuring comfort and quality of life for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.

• Travel daily to patient locations and facilities as assigned.

Qualifications

Valid New Jersey or Multi-State Registered Nurse (RN) license.

Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units.

Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.
