Job summary

MJ Recruiters, LLC is seeking a Hospice RN Clinical Liaison to connect with hospital teams across Bedford and Manchester-area facilities. This role focuses on building referral relationships and facilitating transitions to hospice care, not bedside nursing.

The ideal candidate has strong hospital-based nursing experience and at least 1 year in healthcare sales or marketing, with the ability to navigate discharge planning, physicians, case managers and social workers to grow referrals.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing experience (med-surg).
• Minimum 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver''s license, auto insurance and dependable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Makes regular, planned calls to secure new referrals, build census, and meet established goals with physicians, facilities, case managers, discharge planners and social workers in hospitals and SNFs.
• Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
