Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking an Admissions Registered Nurse (RN) to join our team in Hillsborough County. This PRN role travels across hospitals and in the community to assess eligibility and coordinate care with the Network of Care.

The ideal candidate holds an Associate degree, Florida RN license, and valid Florida driver's license, with 6 months to 1 year of related experience, and a passion for compassionate, patient-centered end-of-life care.

Qualifications

• Associate degree or equivalent from two-year college or technical school.
• Minimum of 6 months to 1 year related experience and/or training; or equivalent.
• Florida RN license and Florida driver’s license.

Responsabilités

• Assess eligibility for Empath Health Network of Care and advocate for appropriate access to care across settings.
• Complete admission process based on care program specifics and patient needs.
• Demonstrate excellent customer service and relationship-building with internal and external stakeholders.

Formation
