Suncrest Hospice in Texas is seeking a Registered Nurse to provide patient care across home, skilled nursing, and assisted living settings. You will join an interdisciplinary team delivering psych-social assessments and case management, admissions, or resource coordination.

Flexible travel across the service area is expected with reliable transportation. The role emphasizes collaborative care, thorough documentation, and adherence to hospice regulations.

Qualifications

• Current RN licensure in the state(s) of practice.
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Able to work independently with phone, email and meetings, while following hospice rules and regulations.
• Strong time management to schedule visits, documentation, and meetings.
• Willingness and ability to drive across the service area with reliable transportation.

Responsibilities

• Provide care and services for a group of patients in homes, SNFs, and assisted living facilities.
• Contribute as part of the interdisciplinary team with psych-social assessments and community resources.
• Work in Case Management, Admissions, or Resource roles.
