Brighton/Suncrest Hospice in Minnesota is seeking a Registered Nurse (RN) to provide compassionate care to a patient group in homes, SNFs, and assisted living facilities. You will join an interdisciplinary team, contribute to psychosocial assessments, and coordinate with physicians, social workers, chaplains, and aides.

Qualifications

• Current Registered Nurse (RN) licensure in good standing.
• Works well in interdisciplinary teams with nurses, social workers, chaplains, and aides.
• Able to travel across service area; reliable transportation and valid driver's license are required.
• Experience with hospice care and understanding of hospice rules and regulations is preferred.

Responsibilities

• Provide care and services for a patient group in homes, SNFs, and assisted living facilities.
• Contribute to psychosocial assessments and interventions as part of the interdisciplinary team.
• Coordinate with physicians, social workers, and other professionals to plan patient care.
• Manage visits and documentation, prioritizing urgent needs and scheduling.
