Job summary

Cynet Health is seeking an experienced Registered Nurse (RN) to provide hospice care in patients' homes on a 13-week contract. You will drive to patient homes daily, handling 5–8 visits per day with a weekly productivity target of 30–35 visits.

Documentation must be completed within 24 hours of each appointment, and travel may extend up to 75 miles from the care center. Required: Active RN license, CPR/BLS, recent hospice or home health experience, and proficiency in OASIS and HomeCare HomeBase

Qualifications

• Active RN license required.
• CPR and BLS required.
• Experience in hospice care required; home health and hospice acceptable.
• Proficiency in OASIS and HomeCare HomeBase EMR used in last 6 months.
• Reliable transportation, a current driver''s license, and agency-required liability insurance.

Responsibilities

• Provide hospice care to patients in their homes.
• Drive to patients'' homes daily; average 5–8 visits per day.
• Meet weekly productivity of 30–35 visits; document within 24 hours.
