Responsibilities

• Implements full-time nursing services to home patients under supervision and agency policies.

• Conducts initial visits to assess physical and emotional status, needs, and resources.
• Prepares nursing care plans and teaches patients per physician plan and standard of care.
• Covers specific areas with a hybrid schedule of visits and on-call duties.
• Visits are scheduled in the early shift hours, with remaining time on-call.
• Evening visits may be required; most visits fall under on-call duties.

Description:Implements the program of full-time professional nursing service to patients in their homes under the supervision of the Clinical Services Supervisor in accordance with agency standards and policies.

Initiates and provides medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition, achievement of rehabilitation potential, and/or reduction of pain and other symptoms.

Makes the initial visit to assess the patient’s physical and emotional status, needs, and resources through observation, interview, and analysis of records.

Prepares the nursing plan of care, in accordance with the physician’s plan of treatment, based on problems identified in the nursing assessment and provides patient teaching using the Care Plan/Standard of Care.

This position will cover specific areas and will involve a hybrid of scheduled visits and on-call duties.

The position consists of scheduled visits during the first few hours of the shift, with the remaining time being on-call.

Evening visits may be assigned, with most visits falling under on-call duties.
