Description:The position involves providing home visits and attending periodic office meetings.The average daily caseload consists of 5 to 8 patients.The weekly productivity expectation includes achieving 30 points.Visit types, documentation, travel, and other activities have different point values.Contractors typically see 5 to 10 patients per day, totaling approximately 6 to 8 points.There are no guaranteed hours available for this contract assignment.If patient census is low, contact the Account Manager regarding potential floating to a nearby clinic for additional hours.Case types may include start of care, wound care, follow-up visits, and other adult home health or hospice services.Local candidates are accepted at the same rate.Clinicians must be proficient in OASIS.All documentation must be completed within 24 hours of the patient appointment.Delayed and incomplete documentation that impacts billing and follow-up patient care could lead to cancellation of the assignment.Candidates must be open to traveling up to 75 miles from the assigned care center.The mileage radius may result in drives longer than 20 minutes between patients.Candidates must be willing to accept every patient on their schedule, regardless of distance.
