Cynet Health is seeking a Registered Nurse specialized in Home Health to provide patient-focused home visits. The role involves Start of Care, wound care, and follow-ups within a 50–75 mile travel radius, with 5–8 patient visits daily.

Requires active RN license, CPR/BLS, and 1 year of recent adult home health or hospice experience. Mileage is reimbursed and documentation must be completed within 24 hours.

Qualifications

• Minimum 1 year of recent adult home health or hospice experience.
• Active nursing license required.
• CPR and BLS certifications.
• Proficiency in HCHB and recent experience within the last 6 months.
• Proficiency in OASIS.

Responsabilités

• Provide home visits for patients in a 50–75 mile radius.
• Typical daily caseload of 5–8 patients.
• Complete Start of Care, Wound Care and Follow-up visits.
• Document visits within 24 hours.
