Qualifications

• Active RN license in the state you work
• Two years of RN experience, Hospice experience preferred
• Current driver’s license and ability to travel ~20% of the day

Responsibilities

• Deliver patient-centered hospice care with an interdisciplinary team
• Assess patients, manage symptoms, plan end-of-life care
• Educate and support patients and families through end-of-life journey
• Think critically and document thoroughly in EMR
• Travel to patient homes within territory
