Graham Healthcare Group seeks aRegistered Nurse (RN) Hospice to provide end-of-life care in patients' homes in McHenry, IL. The role is 40 hours per week with 8-hour shifts and a staff employment type; a $10,000 first-year bonus is offered for milestone achievements.

You will collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life, traveling to patient homes within the assigned territory.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license; ability to drive to patient homes ~20% of time.

Responsibilities

• Deliver patient-centered hospice care in patients' homes.
• Perform comprehensive assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
