Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) to provide Hospice end-of-life care in patients’ homes in Franklin Park, IL. This staff position offers a $10,000 first-year success bonus and a stable 40-hour week.

The RN will work with an interdisciplinary team, perform assessments, manage symptoms, educate families, and ensure compliant EMR documentation while traveling within the territory.

Qualifications

• Active RN license in Illinois.
• Minimum two years RN experience; hospice experience preferred.
• Valid driver’s license; 20% driving required.
• 40 hours per week; day shift; staff employment.

Responsibilities

• Deliver compassionate, patient-centered hospice care at patients’ homes.
• Perform comprehensive assessments, symptom and pain management, end-of-life planning.
• Educate families and support them through the end-of-life journey.
• Maintain thorough EMR documentation and ensure compliance.
• Travel to patient homes within the assigned territory.
