Job summary

The Sequoia Hospice team in Santa Clara County is seeking an experienced Registered Nurse (RN) to join our Hospice Care program. You will plan, organize and direct hospice care in patients' homes, serving as a key case manager to ensure quality, compassionate support for patients and families.

Requirements include an active CA RN license, CPR certification, and at least 2 years of nursing experience (hospice/home health preferred).

Qualifications

• Active California RN license is required.
• Minimum 2 years of nursing experience, hospice or home health preferred.
• Current CPR certification.

Responsibilities

• Plan, organize and direct hospice care; educate patients and families.
• Serve as case manager; ensure quality, compassionate home care.
• Complete initial and ongoing patient and family assessments.
• Develop and revise individualized hospice care plans.
• Provide nursing care using the nursing process, administer meds and treatments.
• Educate and counsel patients and families on hospice care.
• Coordinate care for assigned caseload.
