Graham Healthcare Group in Elgin, IL is seeking a Hospice Registered Nurse to provide compassionate end-of-life care in patients' homes. This full-time, staff position offers 40 hours per week with 8-hour shifts in a field-based setting.

You will collaborate with an interdisciplinary team, perform patient assessments, manage symptoms, educate families, and maintain thorough EMR documentation while traveling to patient locations within the territory.

Qualifications

• Active RN license in IL.
• Two years RN experience; hospice experience preferred.
• Valid driver’s license; ~20% travel to patient locations.

Responsabilités

• Deliver compassionate hospice care to patients at home with an interdisciplinary team.
• Conduct comprehensive patient assessments and manage symptoms/pain.
• Educate families and patients about end-of-life planning and care.
• Maintain thorough EMR documentation and ensure compliance.
• Work independently while collaborating with the team.
• Travel to patient homes within assigned territory.
