• Two years of clinical nursing experience with at least one year of hospice nursing experience preferred.
• Knowledge of home health and hospice care program and reimbursement guidelines.
• CPR certification and yearly continuing education (12 hours).

Responsibilities

• Apply nursing theories and concepts.
• Care documentation meeting reimbursement guidelines.
• Coordinate physical care with patient, family, physician and Hospice team.
• Communicate with patients and families at all levels.
