Job summary
Liberty Hospice, part of Liberty Healthcare Management, in Matthews, North Carolina, seeks an experienced Hospice Admissions Registered Nurse to admit patients and develop individualized plans of care for home-based hospice services.
The role is full-time, Monday–Friday, 8am–5pm, with a $10,000 sign-on bonus. Candidates must hold an active RN license, have at least 1 year of nursing experience, CPR certification, and strong documentation skills for accurate initial visits.
Qualifications
- Requires current RN licensure in the branch state with 1 year of clinical nursing experience.
- Must have completed nursing education from an approved school of nursing.
- CPR certification required.
- Must be able to use a PC and have working knowledge of Windows, Word, Excel.
Responsibilities
- Evaluate and admit patients as assigned according to LCD Guidelines.
- Initiate and develop an individualized plan of care for each patient.
- Coordinate patient care services as necessary.
- Document comprehensive assessment at point of care.
- Work schedule according to HR forms; participate in on-call rotation and staff meetings.
